The name of the images can contain keywords and the "ALT" tag can be used to index your site and improve usability.

When to use it

Its use is opportune on every image of the site. Without it, the pages don't validate at W3C, so they don't appear the same way on different browsers. You are right, many web sites don't validate: big companies expect people to visit because of the name, in spite of their awful web sites. Can you afford that luxury?

Suggestions

Besides describing the image, try to insert a keyword or short keyphrase in your "ALT" tags. The use of the "ALT" tag is good to insert keywords with errors of spelling, but don't insert more than 2-3 keywords per "ALT" tag.
